Gout falls into the TCM category of arthralgia syndrome, and is thought to be caused by pathogens obstructing qi and blood circulation in the channels. During the acute stage it should be treated by clearing heat, dredging the channels, dispelling wind and removing dampness. During the chronic stage it should be treated by removing phlegm, promoting blood circulation, warming the channels and expelling cold. The authors recommend the use of Bai He Yi Yi Ren gruel, which consists of Tu Fu Ling (Smilacis glabrae Rhizoma), Yi Yi Ren (Coicis Semen), Dong Gua Zi (Semen Benincasae Hispidae), Ze Xie (Alismatis Rhizoma), Che Qian Zi (Plantaginis Semen), Da Fu Pi (Arecae Pericarpium) and Fu Ling Pi (Poria Pericarpium). Gout due to damp‑heat can be treated with Towel Gourd gruel, Tu Fu Ling gruel or Fang Ji Yi Yi Ren gruel. Gout due to retention of cold and dampness can be treated with Gan Jiang Fu Ling gruel, Chuan Wu Yi Mi gruel or Du Huo Ren Shen gruel. Finally, gout due to qi and blood deficiency can be treated with Yi Zao Chi Dou gruel or steamed taro noodle with parched sesame. The authors also recommend drinking two litres of water daily, staying away from alcohol and eating foods with a low purine content. (03/10)
